This is a full-time (32-40 hrs.

Travel in and around York County as necessary is required.
Who We AreWoodfords Family Services is a nonprofit organization that's been committed to the support and inclusion of people with special needs and their families in Maine communities for over 55 years! Today, the organization provides an array of clinical, behavioral health and educational programs to more than 2,000 children, youth and adults living with autism, developmental disabilities, intellectual disabilities and/or mental health diagnoses across the state of Maine.
Position SummaryCase Management services are an essential resource for individuals living with intellectual and or developmental disabilities.
They help in identifying consumer needs, connecting and coordinating appropriate services, and supporting navigation through various components of the individual person-centered-plans (PCP).
As a Case Manager, you will serve as a key communicator and advocate for adults and their families by ensuring all areas of development are being met and maintained.
This will be achieved by meeting with adult consumers and the treatment team regularly to identify interests and strengths, vision for their desired lives, and goal setting geared toward attaining the highest level of independence.
Schedule & CompensationFull-time, Monday through Friday (32-40/week).
$40,000 - $44,000 - dependent on education and experience.
Monthly financial incentives starting at $350 minimum!Qualifications Bachelors Degree from an accredited four-year institution of higher learning with a specialization in psychology, behavior health, social work, special education, counseling, rehabilitation, nursing, or a closely related field.
At least one-year's experience in the field of social services.
Valid driver's license and use of a personal vehicle and proof of auto insurance.
